Community Resource Team

The Blaenau Gwent Community Resource Team (CRT) is a joint service provided by Aneurin Bevan University Health Board and Blaenau Gwent Adult and Community Services. The CRT provides support to adults, (over the age of 18) living in Blaenau Gwent to stay as independent as possible within their own homes. Reablement is a short-term programme of intensive support and therapy in your own home. 

The CRT aims to offer a high-quality health and social care service that ensures individuals receive the right intervention, in the right place, at the right time, from the right professional. The team will work with individuals to assess and identify personal goals and outcomes, working with you to achieve these. This may include community physio, falls management, aids, home adaptations, advice and information.

The CRT Multi-disciplinary team consists of:

  • Occupational Therapy
  • Physiotherapy
  • Speech and Language Therapy
  • Health and Well Being Support Workers
  • Assistant Practitioners
  • Reablement Officers
  • Rehabilitation Officer Vision Impairment (ROVI)
  • Falls Service
  • Reablement Service

If you have been assessed as needing reablement support, the service provided will be based on your specific needs and will aim to promote your independence.  

A team member will visit to discuss your goals and schedule daily visits tailored to your needs. As you progress and gain independence, we will gradually reduce our support.

How long will the service last?

This service is provided on a short-term basis and concludes once individual goals are achieved. The duration varies depending on the person, with a maximum period of 6 weeks.
